Eligibility for Dental and Vision Insurance is as follows:

a 2013 IHRC Standardbred License or a 2013 USTA Membership

**Applicants under 18 do not qualify

**Please renew your ISA Membership as soon as possible. ISA members who fail to renew their membership prior to May 1st, 2013 will be removed from the insurance program. ISA Members who join or renew membership after May 1st, 2013 will also be charged a $25.00 enrollment fee and will have a 60 day waiting period for benefits to begin.**

MAXIMUM BENEFIT PER CALENDAR YEAR - $2,000 (NO DEDUCTIBLE)

 

Services covered at 100% up to the maximum benefit:

• Two (2) cleanings per calendar year (once every 6 months)
• Anesthesia
• Fillings
• Fluoride treatments
• Oral exams
• Perio surgery
• Periodontal maintenance (once every 3 months)
• Repair and maintenance of crowns, bridges and dentures
• Root canal
• Scaling and Root planing (per quadrant)
• Sealants
• Simple extractions
• Surgical extractions
• X-rays

Includes:

• One free eye exam per calendar year
• One free pair of glasses or contact lenses per calendar year
  

Insurance will pay for a frame up to $120.00 and a pair of single vision, lined bifocal, line trifocal, or lenticular lenses.

If the frame is over $120.00, insurance will pay 80% over that amount.

 

Insurance will pay for elective, conventional, planned replacement and disposable contact lenses up to $120.00 and will pay 85% over $120.00.
